This study investigated the correlations between clot composition and stroke pathogenesis in 105 patients with large-vessel occlusion. Patients with large artery atherosclerosis had a significantly higher proportion of platelet-rich clots (55.0% vs 21.2%) and platelet content (22.1% vs 13.9%) compared with cardioembolic patients. Patients in the cryptogenic group also exhibited a significantly higher proportion of platelet-rich clots compared with cardioembolic cases. No significant difference was observed between stroke pathogenesis and other major clot components. These results demonstrate an association between high platelet content of emboli and large artery atherosclerosis etiology in patients with large-vessel occlusion.
