Introduction. Stroke is one of the leading causes of death and disability in India and across the world. Apart from the treatment itself, there are several factors that influence the treatment outcome in stroke patients. In addition to higher risk of mortality, the rate of disabilities in stroke survivors, both physically and mentally, is also high.
The objective of the study was to evaluate the socio-demographic and clinical factors that affect the prognosis of stroke patients, and to estimate the incidence and cause of post-stroke depression and anxiety.
Materials and methods. A single centre, cross sectional observational study was conducted in a tertiary care hospital. Data were collected at the time of admission and 30 days after discharge. Mental health was assessed by using perceived stress scale (PSS-10) and hospital anxiety and depression scale (HADS). Pearson correlation coefficient and binary logistic regression analysis were conducted to analyse associations between continuous and categorical variables respectively.
Results. Male patients, those aged above 60 years, widows/widowers, under or overweight patients, alcoholics and tobacco users had higher probability of poor treatment outcome. Patients admitted with haemorrhagic stroke were more likely to have a worse prognosis than those with ischemic stroke. Delayed admission is a major factor affecting the prognosis in stroke. There was a significant positive correlation between PSS-10 scores against HADS-A (r=0.612, p=0.00) and HADS-D scores (r=0.635, p=0.00).
Conclusions. Among survivors, one-third of the patients had depression or anxiety and another one-third borderline depression or anxiety. Post-stroke anxiety or depression were found to have stronger associations with perceived stress.
