The objective of the study was to evaluate the relationship between high-sensitivity C-reactive protein (hs-CRP) level, serum N-terminal pro B-type natriuretic peptide (NT-proBNP) concentration at admission with CHA2DS2 – VASc score and cardiovascular events in patients with non-valvular atrial fibrillation after 3 months.
Material and methods. A cross-sectional follow-up study was conducted on 122 patients with non–valvular atrial fibrillation from May 2023 to June 2023.
Results. The hs-CRP (4.32±2.33 mg/L) and NT-proBNP (1428 pg/mL, IQR: 697.00-5333.00) levels in the high-risk group were significantly higher than in moderate-risk group (2.71±2.36 mg/L and 1028, IQR: 273.50-11311.00 pg/mL, respectively) and low-risk group (0.74±0.39 mg/L and 62.5, IQR: 50.00-112.50 pg/mL, respectively). There was a moderately positive correlation between serum hs-CRP (r = 0.397; p < 0.001), as well as NT-proBNP level (r = 0.383; p < 0.001) and CHA2DS2 – VASc score. In univariate analysis, history of heart failure, CHA2DS2-VASc score, left ventricular ejection fraction, troponin T, hs-CRP and NT-proBNP were among the predictors of cardiovascular events. In multivariable logistic regression analyses, only hs-CRP (OR: 1.483; 95% CI: 1.067-2.062) and NT-proBNP (OR: 1.573; 95%CI: 1.250-1.979) were independent predictors of cardiovascular events. A hs-CRP higher than 4.72 mg/L predicted cardiovascular events with a sensitivity of 76.7% and a specificity of 76.1% (AUC 0.812, p < 0.001). Similarly, with NT-proBNP of 4487 pg/mL and over (sensitivity 83.3%; specificity 91.3%, AUC 0.892, p < 0.001)
Conclusions. In patients with non-valvular atrial fibrillation, hs-CRP and NT-proBNP levels increase significantly, and there is a positive correlation between CHA2DS2-VASc score and these markers in the high-risk group. Additionally, hs-CRP and NT-proBNP can be applied for prediction of cardiovascular events in these patients.
